- Your contact information (name, address, telephone, and email).

- A statement, under penalty of perjury, that the information in the notice is accurate and that you are authorized to act on behalf of the rights holder.
- Your physical or electronic signature.
Counter-notification
If you believe material was removed or disabled by mistake, you may submit a counter-notification with the information required by applicable law, including consent to jurisdiction of the appropriate court and acceptance of service of process from the complaining party.
Repeat infringers
Where appropriate, we may limit access for users or sources that repeatedly infringe copyrights or abuse the service.
